This form should be used to inform the National Office of a change of address.

Note 1: If you are transferring your membership from one chapter to another, you must see your new chapter's President or Membership Vice President to sign and mail the membership transfer application. Also, if you are applying to become a chapter advisor, you must complete and sign the Application for Advisory Membership. You may still use this form to record a change of address, but the necessary applications must still be filed.

Note 2: The Alpha Phi Omega - USA National Office only keeps membership records for members of chapters in the United States. If you are a member of Alpha Phi Omega - Philippines, please click here to update your membership records with their national office.
