This section contains useful documents and forms for chapters (and extension groups) to use in executing their programs and communicating with the National Office, as well as Regional and Sectional Volunteer Staff.

 

Chapter Reporting

 

• Reporting Requirements: This page contains some important things that you need to know about everything that your chapter needs to report to the national office on a regular basis.
  

• Graduating Seniors Report Form: Use this form to report your chapter's graduating seniors to the national office.

• New Officers Report Form: Use this form to report the results of your chapter's most recent elections.

• Service Project Report Form: Use this form to report your chapter's NSW or SYSD service project, or non-participation in either program.

Documents, Applications & Forms 

• APO IMPACT: These documents can be essentially thought of as "instruction manuals" for particular officer positions in a chapter. For more useful documents, be sure to check out the Leadership Resources Clearinghouse in the Leadership area.
  
  
• BANC Application: Build A New Chapter fund - The purpose of the BANC Fund is to support efforts to charter new chapters, to recharter inactive chapters, and to rebuild active chapters considered, "at risk".
  
  
• Event Submission and Update Form: If your chapter is planning a major event and want it listed in the online Calendar of Events, use this form to let us know about it.

• John Mack Scholarship: A scholarship for active members that exhibit exemplary participation and leadership skills in extracurricular activities to benefit those in need, charitable, or humanitarian organizations.

• Pledging Resources: These documents contain everything you need to know about the Pledging process.

• Risk Management Policy: This is the policy in which volunteers abide out of respect for our fraternity, people, property, and laws.
